Successful Latissimus Dorsi Free Flap Salvage using Intraoperative Vasopressors: A Case Report

Arterial thrombosis is a rare, but dreadful complication in microvascular surgery. Here we report on case of recurrent arterial latissimus dorsi (LD) free flap used for scalp reconstruction an 18-year-old woman who sustained gunshot wound (GSW) to the head. In immediate postoperative period, internal and external doppler signals were lost. Emergent surgical exploration revealed thrombosis, which was emergently explored successfully repaired. Doppler again lost second time. Following revision anastomosis, perfusion decreased appeared correlate with patient’s blood pressure. After administration vasopressors increase Mean Pressure (MAP) goal 70 or greater, consistent. describe successful salvage using both intraoperative thrombosis. Furthermore, vasopressor may be beneficial select patients maintain adequate those demonstrate hypotension no other identifiable causes compromise, as present case.
